Questionnaire for language teachers

Dear Sir/Madam,
My name is Karoly (Charles) Novak. I am German language teacher in Hungary but I am learning English, too.
I will write a contrastive essay about teaching German and English grammar in the primary and I have made an international questionnaire for language teachers. I have got many answers from Germany but only a few from English speaking areas yet.
It would be a big help for me if you could send this link from my questionnaire for English teachers with an appeal to fill in it.

Hello Charles, I started, but I hit the wrong key and completed it without finishing it. My impression is that the long list of grammar points to be rated is useless. No decent teacher would try to put those into a hierarchy.
