Question-Mark Exclamations

Is there a rule regarding exclamation points after question marks when someone exclaims, screams or yells, or is it just a stylistic thing? For example:

"What do you want?!" he screamed.

Is it better with or without the exclamation mark, or is it the same thing either way?

The rule is that you can't use two punctuation marks in a row at the end of a sentence like that.

I wish it weren't a rule, but that's what the experts on punctuation say.
