If within the quotation marks there is a grammatical sentence, start with a capital letter.
Your punctuation is almost perfect -- do not leave a space between " and "What" -- however, I can not imagine any real life situation where this sentence can appear.
Did you know "!! What she looks like?"?
Did you know "What she looks like?"?

First of all, "What she looked like" is not a question, and should not have a question mark.

Most style guides will tell you to not use a ? inside a ? - use only one.
