1. What happened to you?

In this sentence, the WH question word is the subject. No auxillary is used.

2. When did it start raining?

In this sentence, the subject is it.
When another word (not the WH - word) is the subject, you must use the auxillary, unless the main verb is "be."

Examples (subject underlined):
What just passed us?
Where did he go?
When is Ramadan next year?
Why is the sky blue?
Why does he work so hard?
