Question about writing addresses

1. In some countries the width of main streets is a part of the street's name. How can write I this in English? example: Kuching 45-m Blvd.? right?

2. Assume Xcompany is located 9 km away from city A along highway B. How can I write the address of the company with this information (X company, Highway B and City A)?
