Question about sentences

Dear all,

I would like to know whether the following sentence is correct. I have seen it in a letter. If it is a passive construction kindly tell me how to form its active form.

"It is seen mentioned that for purchase of CT scan machine funds are to be allotted"

The sentence does not read very well. Without changing the part that you are asking about, this is better:

"It is seen mentioned that funds are to be allotted for the purchase of a/the CT scanner."

As far as your question is concerned, I assume you are asking about "It is seen mentioned" rather than "are to be allotted". Yes this is passive. To make the main verb active you can say
