Punctuation in a Title

This is my title: The Sordid Adventures of Mr. Cody Bishibosh Bryant.

Two questions:

1. Do I put quotes around what is a nickname in "Bishibosh" within the title as I would in some cases of non-title writing?

2. Is there some rule as to whether I have to use either Mr. or Mister there?
