punctuation after a salutation

We are writing a letter and the salutation starts as this,
Dear Antonio Urbina, MD,

Do we put the comma after the MD or leave it off?

I would not start in this way. We don't normally use post nominals when we address people, I would start Dear Dr Urbina, ... .

You can put Antonio Urbina, MD, on the envelope, but don't put it in the salutaton.
Instead say
Dear Antonio, if he is your good friend
or
Dear Dr Urbina,
