It's not correct. I'd write it this way:

I wish (that) smile on the face and glares in the eyes never faded.

I don't think so. I'm sorry I can't say Yes or No, but I can't imagine a use case for having "I wish, <something>."

It would just be "I wish that <something>" or "I wish <something>."
