hit the bull's eye every time = always succeed exactly and completely

Lets change the words and see if it can be more clear:

"If you have nothing planned, you will do nothing"

"If your goals are nothing, you will always reach your goal"

A bulllseye is the center of a target used for archery (bow & arrow), and here it is used to mean 'a goal that you are trying to do'.
