Properly notating unsure spelling

Could someone tell me what the correct way is to notate an unknown spelling of a word or name in a document. I have seen ... (sp), (sp?), or [sic] ... following the word. Are either of these correct or is there another?

org/wiki/Sic means the phrase or word has been copied exactly as originally written, and the copying author wants to make sure people do not think he would make such a mistake. I believe (sp) means that the author himself is not entirely sure about the spelling. This is for informal use only, of course (sp that is, not sic).
