Pronoun agreement

I was told that an adjective clause verb should agree with the subordinator's antecedent.

therefore, I cannot find correction for this sentence.

tom threw me, who is the captain, the ball.

(me is)

Is "me am" any better? "Am" would be grammatical, but everybody I know would stop talking halfway through that, get a pensive look, and start over a different way. Sometimes English is a maze with the occasional blind alley.
