'Program Grid'

Please,

What do we call the system in which viewers have the right to watch several other TV channels because they pay a little bit more per month to watch the programs, news and films they want ? (a closed TV ???). And what is it called the one in which people don't have this privilege?

Finally what do people call the list of programs of a specific channel, for one? By the way is it a "program (or programming ?) grid" ?
