Present perfect continuous with or without time expressions

Is it always necessary to specify the time expression while using Present perfect continuous tense.
Context 1] suppose I am waiting for my friend outside office. Someone ask me 'Why are you standing here?"
I say "I have been waiting for my colleagues."
Is it necessary here to specify the time expressions 'since' or 'for'? Also explain me when to use time expressions & when not.

Context 2] Me & my friend meet at one place. I smell a smoke. What should I ask to my friend? A] Are you smoking? B] Were you smoking? C] Have you been smoking?
If answer is C], why can't other sentences A] & B] be used? & what is the difference? Please explain in detail
