Prepositions for legals

Hi,

I'm writing some kind of legal note for an email and I came up with a doubt, here it is:

... If you have received this transmission IN error, please notify TO PERSON AT +(00) (00) 1234.5678 IN City, State. Country. Thank you.

What do you think is it correct?

Thanks in advanced.
cheers!
