Preposition: on or to?

Sentence: I have reshipped the item to you on/to the address xyz . (XYZ is the address of some person)

Question: To me both prepositions are looking correct. So I am about pretty confused. Please tell me which preposition, to or on, would be correct in the sentence?

Thanks
