Practitioner vs physician vs doctor

What's the difference between a 'practitioner' and a 'doctor' and a 'physician'?

I assume you are asking only about medical contexts. doctor/physician No real difference, except that patients usually say 'doctor' and doctors in my experience usually say 'physiician'. practitioner I only see this as a part of the phrase 'general practitioner (ie family doctor).
