Position of punctuation marks

1. Where do you put period, a question mark, or an exclamation mark? Inside quotation marks or outside?

2. Is it compulsory or optional? For example, which one is right between [ He said, "She isn't involved." ] and [ He said, "She isn't involved". ], between [ He asked, "What do you want?" ] and [ He asked, "What do you want"? ], or between [ He shouted, "Watch out!" ] and [ He shouted, "Watch out"! ]?
