C & C. You need a preposition.

1-3. The boss promoted him (to) (the) personnel direction position by surprise.

2. The boss surprisingly got him promoted (to) (the) personnel director.
O & P, but almost C & P. There's a chance of being misundertood as to who is the personnel director. Also, your use of "got" implies that the boss convinced someone else to promote him.

3. He was/got promoted (to) (the) personnel director.
O & O. However, "the" should be used only if it is clear what precise position he was promoted to, and you should either use "to the" or neither.
