"A poorly functioning sewer could cause backups and overflows" or " A poor functioning sewer may cause backups and overflows"

Is one better than the other, or are they both correct?

You need 'poorly functioning' because you need an adverb of manner, an adverb that answers the question "how?"

How is it functioning? It is functioning poorly.

'could cause', 'may cause', and 'might cause' are all just about equivalent. You can use any of those in your sentence.
