Politely enough

Is it enough politely to write:

"I want to report that I didn't recieve an application form ..."

when I should recieve this application form (they got me know about this in previous email) ?

What you wrote seems to be polite, but nevertheless I wouldn't use the word report; it feels like you

disapprove of them.

I would like to inform you that I haven't received the application form yet.

I would be grateful if you could send me this form at your convenience.
