Police Statement

Sometimes, in the midst of taking a formal, written statement from a suspect or an accused person, I would have to discontinue it (for a while), maybe to give the person (whose statement I am recording) a rest, a toilet-break, etc.

I am, by law, required to indicate why I had stopped recording the peron's statement, and when I subsequently resumed it.

Could someone tell me whether the following clauses, which I intend to use as an explanation for the "discontinuation", are grammatical and idiomatic?

Recorder’s Note: The taking of the statement is discontinued on 5.2.2008 at 1.15 pm for his rest.

Recorder’s Note: The statement is resumed on 5.2.2008 at 1.45 pm. It was recorded by Sgt Sam....

I am particularly disturbed by the parts of the clauses that I have highlighted in red.
