It seems very strange. The only medical-related meaning of "POEM" that I could find is as an acronym for "Patient-Oriented Evidence that Matters." I thought that might be a way of asking for "Cause of Death," but it seems to be a particular category of article in medical journals instead. My guess is that it might mean "Physician or examining medic." That is, a physician or ambulance attendant would often be the person who reports a death. In any case, this line asks for the person who reported the death.
