It seems very strange. The only medical-related meaning of "POEM" that I could find is as an acronym for "Patient-Oriented Evidence that Matters." I thought that might be a way of asking for "Cause of Death," but it seems to be a particular category of article in medical journals instead. There have been questions about the term in other forums, but no satisfying answers. My guess is that it

My guess is that it might mean "Physician or examining medic." That is, a physician or ambulance attendant would often be the person who reports a death. In any case, this line asks for the person who reported the death.
