I've often wondered about the origin of the expression, "to **** someone off," but at least in the US, everyone agrees that it means to make someone angry.
I'm starting to get ****** off.
You're really starting to **** me off.
He's really ******* me off.
He really ****** me off yesterday.

If you tell someone not to **** you off, you really should share in the attempt. You should try to keep your cool, and the other party should try to avoid the things he knows could upset you. When you do become angry, you may be equally responsible.
My point here is that the exact form you use is a bit uncommon. "You'd better not **** me off" would be more likely. It sounds like something a rude parent might say to a child. It's a kind of threat.

Not everyone is angered by the same provocations. The focus is often equally on the person who is becoming angry, and the person who is causing the anger.
But there is not always a living agent. You can be angry at yourself for missing an opportunity, or not seeing the solution to a problem sooner. You can be angry at a threaded nut which doesn't want to start properly onto a bolt.

To goof up is to ineptly make some kind of error.

A goof up may be such an error, or a person who habitually makes them.
