I am writing to refer Mr. DDD to you. Mr. DDD, a 51-year-old married and a heavy smoker, presented at my surgery today, , complaining of pain on 26 and he could not stand the stresses of chewing for last week.



Tooth 26 was Carious. On a small exposure was made. A direct pulp capping was performed and then the tooth was filled by sedative dressing. At that time there were no problems.



Two weeks later the patient came back with mild sensitivity to hot and cold fluids on 26. The patient left our clinic with temporary filling for four months. Six months later as a routine check the same tooth was restored by amalgam, as he a smoker man there was staining on the anterior teeth so I made scaling and cleaning, therefore, the oral hygiene is fair.



On 13/11/07 the tooth was sensitive to heat, few days later Mr DDD came with sever pain examination for tooth was done, the tooth tender to pressure and vitality test negative, sedative dressing and temporary filling were made with antibiotic course.



Could you please examine and treat Mr DDD with appropriate endodontic filling as he wishes to safe his tooth.



Note that Mr DDD has had Rheumatic fever and Hypertension which controlled by medication.
