Plural noun with "whose"

I want to write in a cover letter:

"In a Rapid Response, I use preset protocols and professional nursing judgement to care for patients whose condition is deteriorating."

From what I can find online, it should be, "... to care for patients whose conditions are deteriorating."

That doesn't sound right. Which should it be?
