Pls.help me write a Leave Letter

Hi! everyone! pls help me write a leave letter. I am a Medical technologist and i am going to take a board exam soon. So i plan to leave for a review.

I only have 15 days of leave credits (w/ pay) but i would like to avail 60 days in total of leave. Pls help me. pls

i would like to avail all my leave credits (15 days w/pay) and would like to request an additional of 45 days leave of absence(w/o pay) for my review.

my prospect leave starts frm aug 5 to october 25 (60 days)
