5) Relevant medical certification in xxx, xxx, xxx ...

No. 5 could also be correct in certain situations. And no.6 could be incorrect in some situations. I think it might depend on what the asterisks stand for, and whether the medical certification is a singular document (or process) related to all three of the asterisked elements, or multiple documents/processes that are the same for each one of the three, or a singular document/process that is dif
