Please someone help me revise this letter!

Re: Kings County Juror Summons, Juror Index: 248154953
I am writing this to inform you that I had relocated from the address of 1071 59th St., Brooklyn, NY 11219 to the current address at 31-19 Linden Pl FL2, Flushing, NY 11354, prior to the receipt of the Summons. Due to the relocation, I did not receive the juror summons until an acquaintance forward it to me at my new address – which was already past the deadline. Juror duty is important to me and I did not intend to miss my attendance on the date of the summons. I wish to be rescheduled to attend on another date and please mail the summons to me at the current address provided.
