I've marked the places where I, as a reader, get lost.

What does 'with' go with? "restrictions ... with a list of countries"? I don't understand that part. Or are you trying to connect "considering" with "with" much later in the sentence? That's a stretch.

What does "such a country" refer to? The countries on the "list of countries"? It seems to go in a circle because you're trying to refer to "countries" twice in the same relative clause. I think.

Do you mean "countries after a visit to which a 14-day self-isolation period is required"? If so, there's got to be a better way to say that, and I'm not sure that what you have is that better way.
