Letter Writing
Please help to trim my letter ;)
Hi. I'm a new comer. I'm so happy that I found this website. It's very helpful to a someone who use English as second language. Most of the time, I don't know whether my letter or article write in a proper tone or manner.
Here comes to the case. Please help me to correct it.

We realized that you had undergone internal investigation after the incidents. Please provide with a descriptive detailed reports for the two incidents and they should include:
1. Procedure involved in the concerned diagnosis (e.g. verification of activity, labelling efficiency, etc.)
2. Expected results (please provide wiith a example of expected normal uptake image that you had taken before for the case of I-131)
3. Final results (please provide the ‘no-uptake’ image for the case of I-131)
4. Result of the investigation
5. Corrective and preventive actions regarding to the root cause of the incident
6. Action to the concerned patient after the incident. (e.g. estimation of unplanned patient radiation dose and reschedule another appointment)
We wish to help you from finding out the cause and take proper action avoiding similar incident and preventing any Abnormal Occurrence which may due to similar cause.

Background Information:
"radiopharmaceutical incident" is classified as a minor incident. but "Abnormal Occurrence" should be more sereve.
