Please help me with this sentence

Please help me with this sentence that I received from a co-worker for a form letter we are using.

"It's people like you who care, that makes a difference."

or "It's people like you who care, that makes the difference."

Does "make" need to be plural (makes) to match the word people or does "make" need to match the word "you" (singular)?

Should I take out the "who care"?

It's people like you that make a difference.

Do I need to continue the sentence?

or It's people like you that makes a difference in the lives of children....

or just leave it the way it was?
