please help me to write a letter to general manager

Respected sir,
I would like to inform you that i working here as an accountant from 1st may 2015.but my designation is wrongly mentioned in company salary records " office staff" instead of "accountant.As per companies agreement appointed me as a accountant. so i humble request you to rectify this error as soon as possible.
