Please help me to improve my sentences
Due to severe pain in my teeth, I went to the hospital and doctor told me that two teeth has infected and need to do the treatment of RCS. Doctor said that we would it one by one. We have received a pre-approval with 20% co-insurance for the first tooth from the insurance company. After that we have applied for second tooth, but insurance company has approved with 50% co -insurance. As per our insurance policy, we have dental treatment with 20% co-insurance, not 50%. Why did you charge 50% co-insurance instead of 20% in the policy contract, and what is our total benefit for dental treatment.
Please clarify this matter and advise how we can get the reimbursement for the payment paid additionally.
