Please find attached the medical certificate for Sean who is currently on emergency medical treatment.
His doctor has suggested that he needs a intensive physical rehabilitation for 1 month, so he would take medical leave for 1 month for a complete recovery after surgery.
As per the medical certificate provided, he admitted to the hospital on 15th January 2019 and he got discharged from the hospital on 18th January 2019, so his medical leave starts from 15th January and ends on 16th February 2019.
It is, therefore, requested that his salary be paid for the last month as soon as possible so that he can cover his medical expenses.
