Please help me to correct these sentences

My daughter, Arlene, a student of Class V B of your school, require leave for a period of 14 days with effect from 17/02/2016 that is tomorrow onwards as we are rushing towards our home town due to urgent visit to my mother who is in a critical situation. I kindly request you to grant her leave for 14 days.
