Please help me to correct my sentences

Please look at the below the sentences and help me to correct errors. Thank you for kind help on this matter.


I am writing this to hereby request change of my name, address and nominee in our office record with my new name, address and nominee after dissolution of my marriage and remarriage. After dissolution of my marriage, I changed my name to Ashley Paul which was my maiden name. My name has been unchanged and remained maiden name after remarriage to Mr. Ronald Wilfred. Please update the current name and details on our office record to reflect my new legal name, address and name of the nominee as per below

I have enclosed a copy of marriage certificate and national ID as proof for your ready reference.

I am hoping for your kind consideration with regards to this matter.
