Please help me to correct my sentences

I have claimed for my son's medical claim through your online claim portal under reference number 78218900 on 08/12/2017. On checking the status of my claim online, it is found that the claim has been processed and the cheque was dispatched on 11/12/2017, but I haven't received yet the payment. so please look into this matter and advise.
