Please help me to correct my sentences

As you are aware, one of our staff members, Mr. Kevin is on medical leave for one month due to heart diseas occurred last month. He is now planning to go for his annual vacation from (date) to (date). Hence, please arrange his leave settlement at the earliest so that he can prepare his travel arrangement accordingly.
