Please help me out

I need to write a leave letter for making passport (don't know which is appropriate word). this my first leave letter at my first office. I am very poor in English Emotion: sad Please anyone help me out.

Dear HR

This is notify to you that I may unable to attend office on 01 July'2013 as I am applying for passport and I have to attend at Passport office on this day.
