Please help me correct my grammar, took 8 hours to write a letter

Dear Sir or Madam:
I am writing this letter to appeal for Trillium Drug Program year beginning August 1, 2012 and ending July 31, 2013. I’ve been diagnosed with Lupus in 2008, and I was advised by the pharmacist to apply for Trillium Drug Program because I need to take expensive medication every year.
I sent my application on April 3, 2013. After I sent my application, I went back to china on the same day. Due to my health problems, I was in china for over two months, and I was unable to receive any letters. I stayed with my parents after I came back because I was very sick. Until September I went back home and saw the letter from Trillium Drug Program, I called the call centre on September 25, 2013, and was told to send a letter indicate my income for year 2011. I asked the representative for deadline but no one has told me.
On November 14, 2013 at 2:03PM I called the call centre again to check the status of my application, and I was told the letter that I sent was in wrong year, and was advised to resend the letter on November 14. I sent my second letter on the same day.
No one has told me the deadline for my application until I called on November 21, 2013, and I was told the application is passed deadline. I called several times to find out why I was told to resend the letter on November 14 if the application is already expired. The representatives was very impatient. One of the representative even hung up the phone, and the other told me that I didn’t indicate my income for year 2012. I was very confused and no one has explained it to me until I spoke with the supervisor.
