Please help me check my tenses!

The old man had never fail to bring up a smile on that child's face whenever she saw(?) him.

Should I use the word 'saw' or 'sees' if it had happened over a period of time in the past?

Only context can tell whether the verbs are in an appropriate tense, but 'sees' is not a possibility if we are talking about the past.
