thanks but when the date is not known why should we use underwent? is undergone wrong??

undergone is the past paritciple of undergo and hence it occurs only with auxilary verbs.

Example: He has shown me the letter. He shown showed me the letter.

i am not asking past tense or past participle. i am asking why you have to write underwent when the date is not known??? patient undergo surgery day after tomorrow/patient underwent surgery yesterday/patient have undergone surgery a week days ago. this is the way we use . know my questin is clear i think.

Friend, 'a week ago' is also a time phrase and certainly goes only with past tense.
