Please help!

There's a debate here over whether the word "conform" should end with an "s" in the following sentence:

The purpose of this procedure is to ensure that the orientation of the CHD conform/conforms to the requirements listed in Chapter 3.

With or without the "s". Without the "s" it's a correct subjunctive form, similar to "ensure that this be done". However, most people nowadays use the indicative form with "s", similar to "ensure that this is done".
