Please, grammar check!

Hello!
Could you please check the grammat in this sentence (commas, articles etc.)! I would truly appreciate your help!

On 8th of September 2014 the Ministry of Justice received your submission requesting to contact the relatives of the resident of the Republic of Ireland, Mike XXXX, and to inform them that Mike XXXX consistently provides Dutch authorities and supervising institutions with the home address of the submitter in the Hague (Holland) as his own permanent address, and all the correspondence regarding offences of Mike XXXX is being sent to the aforementioned address.

I don't clearly understand this. You are trying to cram too much information into one terrible sentence. You need to divide it into two or three or four sentences.
