Please, does this sentence look okay?

is this easy to understand?

Before commencing the examination, teacher must receive affirmation that the student has agreed to the examination voluntary without any force, as well as affirmation that the student has no chronic cardiovascular and respiratory diseases and isn’t included in the register of psychoneurologist. Before the examination, student shall receive information regarding code of conduct to be followed during the examination.
