Please correct this sentence native?

Is there problems here?

1/ Can you get me a Medium this shirt. This size is too small. The small is too tight at the shoulders.

I have heard "on the shoulders" is this correct or is "at" correct?

My experience is that both prepositions are used.

"At" seems to answer the question, "Where is it tight?"

"On" seems to imply that pressure is being applied to the shoulders.

It's not an exact distinction, by any means.
