As I said in your prior post, the first is not a sentence. It's just a noun phrase. What is it being used for?
As you requested, the lamp was delivered to Mr. X's office. Please note that, as instructed, cost for the lamp was charged to the SER project. (I'm guessing at the significance of saying it was "for the SER project.")
