Please correct any misused commas.

I would like my sentences to apply to the rules of grammar books.
Here are some sentences of examples where I'm not sure where to put commas, if any at all. (should I put a , between commas if any in the last sentence?)
Okay, here they are:
-How did you do that, shovel, ax, or hammer?
-(Suppose one person says "I need to get a new pair of socks", and the other replies)
"Oh, well, then, you'l; be happy if we go to the store."
-It was lightning out last night. ( is this pernounced "light ning ing" or what?)
-(would it ever be sutable to say) He had had...
-By setting the oven to 350 degrees , and not 450 degrees. ( is a comma needed?)
-This program offers effects and many colors, which aren't found in most others. ( Trying to say that only the colors aren't found in most other programs.)
-(Is this a correct sentence? correct if not) Can you give meeither a pencil or a pen?
-Ha-Ha, that's funny. (a comma should be used after Ha-Ha, right?)
-If not, well, we'll see what happens.
-Why'd I do it, I thought it looked nice. ( should this be - Why'd I do it? I thought it looked nice.?)
-Hi, Michael. (just saying hello to Michael)
-I miss the ocean, living here in Colorado. (instead of - Living herer in Colorado, I miss the ocean.)
-I am Tracy, your flight attendant.
-Contact me, please. ( does a comma go before please when it's at the end of a sentence?)
-That, however is not.
-That to me is a good idea.
-It consists of, like, you know, plastic and wood.
-I like friendly, green-eyed cats.
-This approach known as kneeding, is basically pressing the dough out with the back of your fist. -(or should it be -This approach, known as kneeding, is basically pressing the dough out with the back of your fist.?)
-What, do you think I'm crazy? -OR- What do you think I am, crazy? (can be either, right?)
-I would like my sentences to apply to the rules of grammar books, you know? (should there be a question mark at the end of this sentence?)

The only things I would add to or disagree with are as follows:
-(would it ever be sutable to say) He had had...
This is tricky. need some gurus to answer. By the way, is suitable and not sutable.
In most any situation I can imagine, a second "had" would hardly alter the meaning of your sentence. It is better to re-phrase. Example: He had had a pet. Better: He once ha
