Please correct

Hi;

Is it OK to say " You have been mistaken" (to tell someone that the thing they are doing is not correct/or they made a mistake )

That phrase usually means that someone has not understood something correctly, not that s/he has done wrong. I suggest a simpler 'You have been doing this/that wrong.'
